Minimum Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
• Current, active, and unencumbered license to practice Physical Therapy in the state of assignment (MA) or ability to obtain prior to start.
• Current CPR/BLS certification from AHA or ARC.
• 2-3 years of physical therapy experience preferred. (Client-preferred)
• Experience providing skilled physical therapy in home health settings, including home visits, patient evaluation, plan-of-care development, and documentation.
• Ability to supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned.
• Reliable transportation, valid driver’s license, and agency-required liability insurance for home visits.
• Proficiency with basic computer functions including email, electronic charting, and online training platforms.

Preferred Qualifications

• Experience coordinating discharge planning and working with interdisciplinary teams in home health.
• Familiarity with state-specific home health therapy regulations and documentation requirements.

Assignment Details

• Shift name: Evenings.
• Schedule: 5 shifts per week, 8 hours per shift.
• May require home visits throughout the assigned geographic area. Must have reliable transportation.

Impacting Patient Care

Performs patient evaluations and provides therapeutic services according to physician orders. Responsible to the Director of Operations / Clinical Manager. Provides skilled physical therapy in accordance with the physician’s plan of care, documents patient progress, advises and instructs family and agency personnel, evaluates home environment, and participates in in-service education programs.
