This role requires a clinician who can deliver skilled rehabilitation services and collaborate with interdisciplinary teams to improve patient function and independence.

Minimum Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from a program accredited by the appropriate professional body.
• Active Arizona Physical Therapist license required to start the assignment.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Experience working in a skilled nursing facility or long-term/post-acute care setting preferred.
• Demonstrated ability to perform evaluations, develop treatment plans, and document skilled rehab services.
• Effective communication and interdisciplinary collaboration skills.

Preferred Qualifications

• Previous travel therapy experience preferred.
• Experience with restorative nursing programs and discharge planning preferred.
• Proficiency with electronic medical records preferred.
