Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team supporting home health patients in Santa Rosa, CA. You will help patients recover from injuries and manage chronic conditions while experiencing the lifestyle and attractions of Northern California.

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active California physical therapy license is required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience providing care in home health settings is preferred.
• Ability to perform evaluations and develop individualized treatment plans for adult patients with musculoskeletal and chronic conditions.
• Reliable transportation and ability to travel locally to patient homes in the Santa Rosa region.

Preferred Qualifications

• Previous travel or per diem experience in home health.
• Experience with documentation platforms commonly used in home health agencies.
• Advanced certifications relevant to geriatrics or orthopedics preferred.

Assignment Details

• Shift Days: 5 days per week
• Region: Santa Rosa area; clinician may be expected to travel between patient homes within the region.
• Two Physical Therapists needed for the Santa Rosa region.
