Occupational Therapist - SNF/LTC

Key Responsibilities

• Complete functional assessments and develop individualized treatment plans for residents
• Provide therapeutic interventions focused on activities of daily living (ADLs), mobility, and safety
• Communicate resident progress and recommendations with the interdisciplinary team and families
• Collaborate with COTAs and Rehab Aides to ensure consistent, high-quality care
• Foster a positive, resident-centered rehabilitation environment

Required Qualifications

• Degree from an accredited Occupational Therapy program
• Active Oregon Occupational Therapy license or eligibility to obtain
• CPR certification or ability to obtain prior to start
• Strong communication, clinical reasoning, and teamwork skills

Preferred Experience / Skills

• Experience in a skilled nursing facility (SNF), post-acute, or long-term care setting
• Familiarity with Nethealth documentation software
