We are seeking a dedicated Home Health Physical Therapist to join our team. This role involves providing physical therapy services to patients in their home settings. The ideal candidate will have relevant experience in home care and proficiency in utilizing Homecare Homebase and OASIS documentation.

Job Responsibilities

• Conduct comprehensive assessments of patients in their home environment.
• Develop and implement individualized treatment plans tailored to each patient's needs.
• Utilize adaptive and assistive devices to enhance patient mobility and safety.
• Educate patients and their families on home exercise programs.
• Monitor and document patient progress using OASIS standards.
• Collaborate with interdisciplinary teams for holistic patient care.

Qualifications

• Minimum of 2 years of experience in home care settings.
• Current Physical Therapist license in DC is required.
• BLS certification is required.
• Experience with Homecare Homebase and OASIS documentation.
• Proficient in working with the geriatric and orthopedic patient populations.
• Ability to operate various equipment, including gait belts and Hoyer lifts.

Work Schedule

This position requires availability for day shifts, typically 5 days a week, from 08:00 AM to 05:00 PM. Flexibility to float to additional zip codes based on patient needs and staffing availability is essential. No weekend or on-call requirements will be part of this role.

Compensation

Mileage reimbursement is provided at a competitive rate for travel between patient visits, starting from the first patient of the day. Contractors are expected to use their personal vehicle, maintaining all necessary licenses and insurance requirements.

Patient Population

The Home Health Physical Therapist will primarily work with adult and geriatric patients. A maximum of 6 visits per day is expected, and appropriate support services are available, including social services and interpretation services.
