Job Description

We are seeking a dedicated Home Health Physical Therapist to provide high-quality care to patients in their homes. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for improving patient outcomes, especially in geriatric and orthopedic populations.

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Implement therapeutic exercises and interventions to enhance mobility and function.
• Educate patients and their families on home exercise programs and the use of assistive devices.
• Maintain accurate documentation in accordance with OASIS and HomeCare HomeBase standards.
• Assess and address skin integrity, pain management, and body mechanics.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Float to cover additional zip codes as needed based on staffing requirements and patient needs.

Qualifications

• Current Maryland Physical Therapy license.
• Minimum of 2 years of home health experience, with recent knowledge of HomeCare HomeBase and OASIS.
• Basic Life Support (BLS) certification.
• Experience in geriatrics and orthopedics preferred.
• Proficient in using adaptive and assistive devices.
• Understanding of chronic conditions such as COPD and CHF.
• Ability to utilize mobility aids, including gait belts and Hoyer lifts.

Work Environment

This position involves providing home health physical therapy services in various residential settings. Clinicians will use their personal vehicles and must maintain reliable transportation in good standing, including all necessary licenses and insurance.

Additional Information

• No weekend or on-call requirements.
• Service areas include Montgomery County, Prince George’s County, Charles County, and St. Mary’s County.
• All mileage for travel between patients is reimbursed.
