All applicants must have at least 1 year of certified home care experience and 1 year of case management experience. Please note that experience in hospice, private care, or working as a case manager independently does not qualify as certified home health care management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Applicants must be able to travel to assigned cases, with weekend rotations occurring once every 6 weeks.

Job Summary

As a Home Health Registered Nurse (RN), you will be responsible for:

• Assessing patient conditions
• Implementing care plans
• Providing ongoing education and support to patients and their families
• Delivering patient care within the organizational philosophy, policies, and standards of community health practice
• Performing admission processes and case management responsibilities in accordance with organizational policies and standards of practice
• Utilizing thorough and timely electronic documentation and interdisciplinary communications

Qualifications

• Minimum 1 year of certified home care experience
• Minimum 1 year of case management experience
• Ability to travel to cases as assigned

Start date will be based on credentialing clearance. If credentialing is not cleared by the posted start date, a rescheduled start date will be necessary.

This position is expected to cover multiple locations within the region, specifically areas including Chelsea, East Boston, Revere, and Winthrop.
