Description

Home Health Registered Nurse

We are seeking a dedicated Home Health Registered Nurse (RN) to provide exceptional patient care in a home setting. The ideal candidate will bring a wealth of experience in performing OASIS assessments and care planning for in-home patients.

Responsibilities

Deliver high-quality nursing care to patients in their homes.
Conduct OASIS assessments in accordance with established guidelines.
Develop and implement comprehensive care plans tailored to individual patient needs.
Coordinate with healthcare professionals to ensure optimal patient outcomes.
Participate in on-call rotation, providing support at night, weekends, and holidays as required.

Qualifications

Minimum 2 years of experience in home health care with a focus on OASIS assessments and care planning.
Previous experience with electronic health record systems (Epic preferred).
Must be a licensed Registered Nurse in the state of practice.
Ability to work independently and manage time effectively in a home health environment.

Work Schedule

This position offers day shifts from Monday to Friday, with the requirement to participate in an on-call rotation for nights, weekends, and holidays (one in four).

Additional Information

The position is available for full-time hours ranging from 32 to 40 hours per week.
