Summary:
This role involves filling orders for medications, monitoring patient drug therapies, and providing essential drug information. The pharmacist will deliver pharmacy services with a commitment to compassion, excellence, and efficiency.
Responsibilities:
Operational Duties and Responsibilities:
- Prepare and dispense drug orders per physician request according to established protocols.
- Interpret drug orders (verbal and written) and accurately transcribe them into computerized patient medication records, maintaining an accurate drug record.
- Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products.
- Issue controlled substances to patient care areas and maintain records in compliance with legal and institutional policies.
- Provide services efficiently and timely.
- Maintain a current pharmacist license and ensure all continuing education records are filed in departmental files.
- Attend staff meetings and complete all competency assessments as required.
- Supervise pharmacy support personnel and verify the daily activities of pharmacy technicians.
- Provide ambulatory pharmaceutical care services, including drug therapy management and patient education.
Clinical Duties and Responsibilities:
- Ensure safe, appropriate, and cost-effective drug therapies for patients according to established guidelines.
- Monitor drug therapy regimens for contraindications, drug interactions, allergies, and appropriateness of drug and dose.
- Assist with pharmacokinetics consult services and renal drug dosing as per hospital protocols.
- Accurately read, extract, and interpret information from patient medical records.
- Detect and report suspected adverse drug reactions promptly.
- Sustain the drug formulary, minimizing non-formulary procurements and promoting rational drug therapy.
- Provide clinical consultations and clarifications to practitioners as needed.
- Offer accurate drug information to the hospital’s professional staff.
- Educate patients and their families regarding medications according to institutional protocols.
- Participate in quality improvement and medication use review activities, collecting data and maintaining relevant documentation.
- Engage in designated interventions as defined in departmental activities, reporting clinical productivity as per departmental goals.
- Participate in antibiotic stewardship, opioid stewardship, and other management activities as assigned.
- Attend interdisciplinary rounds when assigned.
Occupational Hazards:
- Potential exposure to hazardous substances, including chemotherapeutic drugs and cleaning solutions.
- Risk of exposure to needlesticks and other sharp objects.
- Potential for musculoskeletal injuries without proper lifting techniques.
- Potential exposure to biologic material during direct patient care activities.
Job Requirements:
Education/Skills:
- Doctor of Pharmacy or BS Pharmacy degree required.
Experience:
- Hospital experience preferred.
Licenses, Registrations, or Certifications:
- State pharmacy licensure required.
- BLS or ACLS certification required.
- PALS certification required for pediatric practice.
