Position Overview

The Occupational Therapist is a key member of the interdisciplinary team responsible for developing, administering, and implementing therapeutic services to help patients regain their optimal level of function in meaningful daily activities, including self-care, leisure, and work. This role is supervised by the Home Health and Hospice Manager.

Essential Job Functions

• Evaluate patients' functional status and interpret and implement the physician's plan of care.
• Conduct assessments and reassessments, making decisions based on objective tests and measurements.
• Communicate observations, facts, and treatment recommendations with the interdisciplinary team, both individually and in team meetings, to ensure coordination of the care plan.
• Provide education and guidance to patients and coworkers as appropriate.
• Work both independently and collaboratively with team members.
• Recognize and uphold patient rights.
• Demonstrate empathy and compassion towards patients experiencing grief, pain, or trauma.
• Maintain accurate and concise documentation of all communications, coordination, and treatment activities.
• Adapt to emergency situations and remain alert to changes in the patient's condition.
• Support the organization's mission, vision, values, policies, and procedures.
• Participate in required educational programs relevant to the position.
• Assist in staffing needs by floating to other departments as qualified.
• Perform other related duties as assigned.

Qualifications

Education

• Required: Graduate of an occupational therapy curriculum accredited by the Committee on Allied Health Education and Accreditation and American Occupational Therapy Association; eligible for the National Registration Examination.
• Desired: Evidence of continuing education relevant to skill level and area of practice.

Licenses/Certifications/Registrations

• Required: Licensed by the State of Oregon as an Occupational Therapist, valid driver's license, current certification in BLS, and proof of auto insurance.
• Preferred: Registration as an Occupational Therapist with the American Occupational Therapy Association.
• CRASE training is required if travel assignment exceeds six months.
