We are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will be responsible for providing high-quality care to patients, focusing on promoting optimal functional independence and safety. The following outlines the key responsibilities and requirements of the role:

Responsibilities:

• Actively manage and adjust patient visit schedules to optimize productivity and address patient needs, including rescheduling visits and communicating changes to supervisors.
• Conduct comprehensive assessments of patients, covering psychosocial, educational, safety, and risk factor parameters in accordance with relevant guidelines and regulations.
• Develop individualized plans of care that promote optimal functional independence and safety, ensuring they reflect logical interventions and measurable goals.
• Implement skilled physical therapy treatments and provide patient and caregiver education, while regularly assessing patient progress and quality of care.
• Coordinate patient care with other clinical team members, ensuring effective communication to facilitate optimal patient outcomes.
• Supervise and direct the clinical activities of Physical Therapist Assistants (PTAs), Physical Therapy students, and home care aides.
• Complete required documentation accurately to reflect care provided and outcomes achieved, adhering to regulations and standards.
• Ensure accurate execution of billing processes, including completion of necessary insurance forms and addressing coverage inconsistencies.
• Participate in in-service training programs for healthcare staff and community groups as needed.
• Engage in continuous professional development and uphold standards in physical therapy practice.
• Promote rehabilitation services; participate in community health initiatives and marketing efforts.
• Conduct various patient-related health assessments and procedures, ensuring compliance with safety protocols.
• Communicate schedule changes and patient information promptly to the scheduling team and supervisors.
• Complete evaluations within mandated timeframes, adhering to agency and state guidelines.
• Collaborate with Quality Improvement (QI) staff to enhance clinical documentation and accuracy of records.
• Follow agency protocols for visit documentation and reporting.
• Efficiently conduct patient visits, ensuring timely communication with the scheduling department to manage visit types.
• Assess patient needs for Home Care Aide (HCA) services and provide necessary supervision in line with regulations.
• Coordinate safe patient discharges and transfers, ensuring all documentation is processed correctly.
• Timely transmission of patient incident reports to QI Specialists when necessary.
• Participate in peer review audits and provide and receive constructive feedback professionally.

Shift Information:

Full-time position, Monday to Friday, from 8 AM to 4 PM. Weekend coverage of 2-3 days may be required. Holiday coverage is also necessary.

Experience Requirements:

A minimum of 2 years of experience in a related field is required.
