Position Overview
The Case Manager assesses, develops, implements, and monitors a comprehensive plan of care through an interdisciplinary team process in collaboration with patients and their families. This role encompasses internal and external settings, focusing on the holistic evaluation of patients' biophysical, psychosocial, environmental, functional, and financial status.
Essential Job Functions
- Assess patients' healthcare needs and formulate comprehensive care plans.
- Facilitate communication between patients, families, and healthcare providers.
- Coordinate transitions between various care settings (e.g., hospital to home, rehab, long-term care).
- Ensure continuity of care throughout the treatment process.
- Evaluate medical necessity for treatments, procedures, and hospital stays.
- Collaborate with insurance companies to obtain necessary authorizations.
- Monitor patient length of stay and identify potential discharge barriers.
- Advocate for patients' needs and preferences within the healthcare team.
- Educate patients and families on diagnoses, treatments, and self-care practices.
- Connect patients with community resources and support services.
- Address barriers to care including financial, social, and logistical challenges.
- Regularly review and update care plans based on patient progress.
- Facilitate referrals to specialized services (e.g., housing, healthcare, legal aid).
- Maintain accurate and thorough case management records and documentation.
- Track quality metrics and patient outcomes, preparing reports for healthcare teams.
- Work collaboratively with physicians, therapists, social workers, and other healthcare team members.
- Participate in interdisciplinary care conferences and consult with specialists regarding complex patient needs.
Qualifications
- Education: Graduate of an accredited school in medicine, social work, nursing, or a related medical occupation. A BSN or MSN is preferred.
- Licenses/Certifications: Active Oregon RN License and CRASE training are required. Case Management Certification is preferred.
- Experience: Previous clinical experience in an acute care or home health setting, including discharge planning and/or case management, is required.
- Additional Skills: Bilingual proficiency in English and Spanish is preferred, with a strong ability to work with a culturally diverse population.

The position involves physical coordination for medical tasks and may require occasional lifting and ability to manage patients weighing over 100 pounds with proper techniques.

This position is primarily in an indoor healthcare environment, with exposure to various health-related risks. The role requires adherence to safety protocols while being able to respond quickly to a fast-paced and sometimes unpredictable work environment.
