Description

Physical Therapist (PT)

Job Description

We are seeking a dedicated and experienced Physical Therapist with expertise in home health care and hospice services. The ideal candidate will have a minimum of 2 years of relevant experience, including proficiency with OASIS assessments and travel for home visits.

Responsibilities

Conduct patient evaluations and develop individualized care plans.
Provide outpatient physical therapy services in home settings.
Manage and adjust schedules to accommodate patient needs and changes.
Collaborate with healthcare teams to facilitate patient recovery.
Utilize the HealthWyse electronic medical records (EMR) system for documentation.

Qualifications

Must possess OASIS experience.
Home health and hospice experience required.
Ability to travel between patient locations for home visits.
Flexible and adaptable to changes in scheduling.
Strong communication and interpersonal skills.

Schedule

This position is full-time, Monday through Friday, from 8:00 AM to 5:00 PM, with holiday coverage as needed.
