Job Details

The Hospice Nurse will provide compassionate care and support to patients in a home health or hospice setting. This role involves working closely with patients and their families to ensure comfort and quality of life during end-of-life care.

Shift Information

• Shift Type: Days
• Shift Length: 12 hours
• Guaranteed Hours: 36 hours per week
• Schedule: 3 shifts per week from 08:00 to 20:30

Qualifications

• Years of Experience: Minimum of 2 years
• Required Certifications: BLS (AHA)

Unit Information

This position is within the hospice unit, focused on delivering high-quality care to patients in their homes. The Hospice Nurse will play a key role in ensuring that patients receive not only medical care but also emotional and spiritual support.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor patient conditions and provide updates to the healthcare team.
• Educate patients and families about caregiving and health management.
• Document patient progress and plan of care in compliance with standards.
• Coordinate with interdisciplinary teams to ensure comprehensive care.
