Job Description

We are seeking dedicated Licensed Vocational Nurses (LVN) / Licensed Practical Nurses (LPN) to provide quality care in a long-term care setting. The ideal candidate will be committed to improving the lives of residents and ensuring their well-being.

Key Responsibilities

• Continuously observe residents, noting any changes in mental and physical health, and report information to the Supervisor.
• Accurately receive, transcribe, and implement physician's orders.
• Administer medications and treatments as prescribed, ensuring proper documentation via a computerized system.
• Supervise nursing assistants in accordance with licensing requirements, assigning care responsibilities appropriate to their skills and overseeing their performance.
• Accompany physicians on rounds to address residents’ medical needs while facilitating effective communication.
• Provide oxygen and feeding via gastrostomy/jejunostomy as per facility guidelines, and perform wound care as needed.
• Manage daily unit tasks, including appointment checks, monitoring nutrition charts, and ensuring adherence to infection control practices.
• Handle admissions, discharges, and transfers, ensuring accurate documentation is completed.
• Prepare and present 24-hour reports for assigned units and maintain open communication with nursing leadership regarding resident care and staffing issues.
• Engage with residents' families to address concerns and communicate treatment updates.
• Receive incoming calls and provide appropriate responses.
• Oversee unit management during assigned shifts, ensuring compliance with state and federal regulations.
• Assist in the orientation and training of new nursing staff.

Qualifications

• Minimum of 3 years of current experience in a long-term care setting within the last 6 months.
• Must be willing to float between various units based on experience.
• IV certification is required (candidates must be willing to obtain certification during the onboarding process if not already certified).
• COVID-19 vaccination is mandatory; declinations and exemptions are not accepted.
• Must reside beyond 50 miles from the facility.
• Previous travel experience and experience in rural or small facilities are highly preferred.

Certification Requirements

• BLS certification (American Heart Association).
• Active New Hampshire nursing license required upon submission.
