Position Overview

The Hospice Nursing Case Manager will be responsible for managing the care of patients in a home hospice setting. The role requires a dedicated and compassionate professional to ensure the highest quality of care for patients and their families during challenging times.

Key Responsibilities

• Develop and implement comprehensive care plans tailored to individual patient needs.
• Coordinate with interdisciplinary teams to provide holistic care for patients and families.
• Conduct regular assessments and evaluations to monitor patient status and adjust care plans as necessary.
• Educate patients and families about hospice services and end-of-life care options.
• Maintain accurate and timely documentation of patient care and progress notes using EPIC.
• Ensure compliance with all regulatory standards and organizational policies.

Qualifications

• Current and valid Registered Nurse (RN) license.
• Experience in home hospice case management is required, with no exceptions.
• BLS certification is required.
• Proficiency in EPIC electronic health record system is mandatory.
• Excellent communication and interpersonal skills to interact effectively with patients and their families.
• Ability to work independently and as part of an interdisciplinary team.

Work Schedule

Day shift, five days a week from 8:00 AM to 4:30 PM. This position does not involve block scheduling.
