Description

Physical Therapist

We are seeking a qualified Physical Therapist for a position within a skilled nursing facility. This role requires a dedicated professional with prior experience in a similar setting.

Key Responsibilities

Provide direct patient care and support rehabilitation processes.
Develop and implement individualized treatment plans based on patient assessments.
Conduct evaluations and documentation of patient progress.
Collaborate with interdisciplinary teams to optimize patient outcomes.
Educate patients and their families on the rehabilitation process and techniques.

Qualifications

Valid Physical Therapist license in New York.
Current Basic Life Support (BLS) certification.
Prior experience working in a skilled nursing facility is required.
Strong communication and interpersonal skills.
Ability to work effectively in a team-oriented environment.

Work Schedule

Monday to Friday, from 8:00 AM to 4:30 PM.
