The Physical Therapist (PT) is responsible for evaluating the physical therapy needs of assigned patients and establishing goals to develop and implement tailored physical therapy programs aimed at enhancing patient mobility and independence. The role demands effective interaction with patients, physicians, and a multidisciplinary healthcare team, while providing age-appropriate care to a diverse patient population ranging from pediatric to geriatric.

Key Responsibilities

• Assess, plan, treat, evaluate, and implement a plan of care (POC) for all clients seen.
• Demonstrate a thorough understanding of the admission process, treatment costs, coding, financial support, and appropriate billing.
• Communicate effectively with patients, families, physicians, and staff, embodying the organization’s core values.
• Complete documentation of all patient care activities promptly in the electronic medical record.
• Assist in the transfer and positioning of patients as required.

Qualifications

• Minimum of 1 year of home health physical therapy experience is required.
• A valid driver’s license and car insurance are required.
• COVID vaccination is required; declinations will not be approved.
• Experience with OASIS and start of care documentation is strongly preferred.

Certification Requirements

• Basic Life Support (BLS) certification is required.
• An active state license is required at the time of submission.
