Job Description

We are seeking a dedicated Physical Therapist (PT) for a home health position. The primary responsibility will be to provide physical therapy services to clients, focusing on direct patient care, collaboration with team members, and participation in developing comprehensive plans of care.

Key Responsibilities

• Deliver direct physical therapy services to clients in a home health setting.
• Collaborate with other healthcare professionals to enhance patient care.
• Contribute to the development and implementation of individualized treatment plans.
• Utilize rehabilitation equipment such as splints, orthoses, and modalities.
• Maintain accurate documentation of patient progress and treatment.

Qualifications

• 2+ years of acute nursing care (hospital-based) and home care experience.
• Must have a valid Driver's License.
• Ability to work independently with minimal supervision.
• Home health certification, with proficiency in OASIS, is required.
• Active Massachusetts Physical Therapist License.
• Current American Heart Association BLS certification.

Work Schedule

This position offers 8-hour days with a 40-hour guarantee. Rotating weekends and holiday coverage may be required as needed.

Additional Information

Candidates must be prepared for a non-billable 8-hour orientation and meet all necessary fit testing requirements. Mileage reimbursement will be provided for travel between designated office locations only, not to or from personal residences.
