Job Description

The primary focus of this position will be in the Rehab unit, with responsibilities to float to Med/Surg as necessary.

Essential Duties and Responsibilities

• Supports and promotes the mission and values of the healthcare organization.
• Provides professional nursing care to patients within an assigned unit and carries out providers’ medical orders.
• Ensures comprehensive clinical documentation including care plans is completed each shift.
• Seeks and responds to feedback regarding nursing practices.
• Communicates patient information effectively for continuity and safety through bedside shift report, multidisciplinary rounds, and hourly rounding using AIDET.
• Maintains compliance with unit-specific competencies and requirements.
• Completes all required learning modules within the designated timeframe.
• Promotes a culture of well-being, contributing to a positive atmosphere each shift.
• Provides a safe environment for patients, visitors, and staff by following established procedures and protocols.
• Identifies and assists in correcting patient care safety issues.
• Works within the guidelines of a Just Culture Environment to encourage error reporting and safe practices.
• Participates in daily unit-based safety huddles and reports issues through the appropriate chain of command.
• Stays updated on unit-based nurse-sensitive indicators, including infection rates and patient safety metrics.
• Engages in quality initiatives and performance improvement efforts to enhance patient outcomes.
• Collaborates with nursing colleagues across departments to leverage diverse clinical expertise.
• Assists all team members and actively participates in shared governance.
• Develops and implements performance goals based on self-assessment and feedback.
• Ensures patient education materials are culturally competent.
• Serves as a preceptor for new nurses and engages in peer feedback processes during annual reviews.
• Participates in maintaining survey readiness for accreditation surveys.
• Effectively delegates aspects of care and unit activities to nursing personnel.
• Collaborates in planning, supervising, and instructing nursing students and assistants.
• Maintains current nursing licensure and certifications required for the department.
• Participates actively in employee engagement surveys.

Qualifications

• Current RN license in New Hampshire is required.
• Graduate of an approved nursing program is preferred.
• Basic Dysrhythmia knowledge is preferred or must be obtained prior to orientation completion.
• Crisis Prevention Institute (CPI) training preferred or must be completed by the end of orientation.

Additional Information

Holiday coverage is required for this position. All candidates must manage their time off in accordance with policy guidelines.
