Shift Information
The Pharmacist will work 40 hours per week on varied shifts. Candidates must be willing to cover weekends and evening shifts. Typical shifts will be from 12:30 PM to 9 PM, with weekend hours varying based on staffing needs.
Job Responsibilities
- Provide comprehensive pharmaceutical care to patients, including medication management, safety, and efficacy evaluations.
- Utilize pharmacokinetic dosing, particularly for vancomycin.
- Manage warfarin therapy, ensuring optimal patient outcomes.
- Collaborate with healthcare professionals to optimize medication therapies.
- Ensure compliance with regulatory requirements and standards of care.
Qualifications
- Active Pharmacist license in Ohio in good standing.
- Minimum of one year of inpatient hospital pharmacy experience required.
- Experience with Epic Willow Inpatient is preferred but not required.
Additional Information
Holiday coverage is required for this position.
