Job Description

We are seeking a dedicated Physical Therapist (PT) to provide treatment for illness and injury through physical exercises and therapies. The primary goal is to assist in the rehabilitation and restoration of bodily functions, promoting maximum wellness and independence for patients of all ages with developmental deficits, physical injuries, or illnesses. Additional responsibilities may vary based on facility needs.

Responsibilities

• Coordinate physical therapy services, offering direct therapeutic care and teaching rehabilitation skills to clients and caregivers to promote independent functionality and safety.
• Provide individualized preventive and rehabilitative physical therapy procedures tailored to client and age-specific needs.
• Develop and revise plans of care based on initial assessments, continual reassessments, client history, diagnosis, physician orders, expected outcomes, and client goals, including discharge plans as necessary.
• Apply treatments using physical agents such as heat, water, and electricity, utilizing equipment like whirlpool baths, electrical muscle stimulation, and ultrasound.
• Follow established utilization guidelines and protocols.
• Formulate and implement teaching plans, ensuring significant clinical findings and changes are communicated to physicians and other personnel involved, including clients.
• Document all skilled and medically necessary care thoroughly, timely, and accurately, ensuring compliance with Medicare, State, Federal regulations, JCAHO, insurance, and payer requirements, as well as local policies.
• Maintain client and organizational confidentiality at all times.
• Participate in the Quality Management program and complete all required in-service training programs to uphold standards and ensure skills are current.
• Conduct supervisory visits for Physical Therapy Assistants and Home Health Aides as applicable, preparing and updating written instructions in accordance with local policies.
• Participate in staff meetings, case conferences, business development meetings, and customer service programs as required to help the facility meet its objectives.
• Deliver exceptional customer service to all internal and external customers.
• Exhibit leadership competencies through effective communication, relationship building, and teamwork.
