Job Summary:

The Home Health Registered Nurse (RN) is responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. This role demands a commitment to delivering patient care aligned with organizational philosophy, policies, and community health practice standards. Key responsibilities include performing admission processes and managing cases as per organizational guidelines, while ensuring thorough and timely electronic documentation and effective interdisciplinary communication.

Key Responsibilities:

• Assess patient health conditions and needs.
• Implement individualized care plans based on patient assessments.
• Provide ongoing education and support to patients and their families.
• Perform admission processes in compliance with organizational policies.
• Carry out case management responsibilities effectively.
• Utilize proficient electronic documentation and maintain communication with the interdisciplinary team.
• Participate in weekend rotations, occurring once every six weeks.

Qualifications:

• Minimum of 1 year of certified home care experience required.
• At least 1 year of case management experience required.
• Ability to travel to patient cases as assigned.
• Experience must be gained specifically within a certified home health agency; roles in hospice or private care do not qualify.
