The Physical Therapist will perform evaluations, create individualized treatment plans, and support patients in enhancing their mobility and functional activities. The therapist will work collaboratively to restore function, reduce pain, and prevent disability in a long-term care environment.

Essential Functions and Responsibilities:

• Conduct comprehensive patient evaluations by obtaining medical history, performing relevant system reviews, and administering specific tests and measures to gather data.
• Develop individualized plans of care that address the specific impairments identified during evaluations and are aligned with physician orders.
• Perform ongoing assessments of patient progress and response to treatment.
• Initiate and manage the discharge planning process from the first visit through the duration of treatment.
• Supervise and delegate responsibilities to ancillary staff and students, following state laws and departmental policies.
• Ensure accurate and clear documentation within the timeframe established by departmental policies and in accordance with accreditation and insurance regulations.
• Communicate effectively with patients, families, caregivers, and healthcare team members to maximize care outcomes.
• Assess the educational needs of patients and their support systems, providing necessary education accordingly.
• Engage in professional development by participating in educational programs, workshops, and current literature related to practice scope.
• Actively participate in quality improvement initiatives.
• Maintain a safe environment for patients, visitors, and staff while complying with safety standards.
• Operate within the scope of practice as defined by state regulations.
• Perform additional duties as required and directed.
