Job Description
We are seeking a qualified Occupational Therapist (OT) to join our home health team. The ideal candidate will demonstrate expertise in treating adults and geriatric patients in their home environments.
Position Details
- Shift: 5x8 hours
- Patient Load: 8-12 patients assigned
- Weekend Requirements: No
- First-timers Accepted: Yes
- Floating: Required within scope to meet facility needs
Qualifications
- Active and unrestricted state Occupational Therapy license
- National certifications required: MOAB, NBCOT
- BLS certification
- Mandatory second set of fingerprints upon arrival for CYFD (for home health services involving children)
Additional Information
- MOAB Certification: Required; if not already certified, the candidate will receive a 4-hour de-escalation course during the first week. Failure to pass may result in cancellation of contract.
- Local Candidates: Not accepted within a 100-mile radius.
- COVID Vaccine: Optional; required reporting of COVID vaccination status must be completed upon assignment.
- Productivity Points: 27 points for a 40-hour week, with varying weights for each visit.
- Mileage Reimbursement: At the federal IRS standard mileage rate, for a territory of 35 miles.
Contract Details
- The duration of new contracts and extensions must be at least 10 weeks.
- A mandatory 6-month break period is required after 4 consecutive assignments or upon reaching one year of service, whichever comes first.
