Job Overview

We are seeking a dedicated Physical Therapist (PT) to provide home health services to patients in the designated counties. The ideal candidate will work primarily with adults and geriatric populations, delivering therapy focused on improving mobility, strength, and daily functional activities.

Responsibilities

• Conduct comprehensive patient assessments to determine individual therapy needs.
• Develop and implement personalized treatment plans aimed at enhancing patient mobility and quality of life.
• Provide therapy for a variety of conditions, including joint replacements, COPD, CHF, and T2DM.
• Complete necessary documentation and maintain accurate patient records using Home Care Home Base (HCHB) and eventually EPIC.
• Coordinate with other healthcare professionals to ensure comprehensive patient care.
• Participate in ongoing professional development and training as required.

Qualifications

• Minimum of 1 year of experience as a Physical Therapist; first-time travelers are welcome to apply.
• Valid SC License or Compact PT License eligible, with proficiency in BLS certification.
• Experience in home health care is highly preferred; online training in Home Care Home Base is available for those new to the system.

Work Setting

• Role involves travel within designated counties with an average daily travel of 20-30 miles; anticipated maximum of 70 miles per day.
• Manage a patient caseload with an average of 6 visits per day, working towards a productivity benchmark of approximately 25 units.

Schedule

• Full-time, day shift (5 days a week, 8-hour shifts).
• Availability to work holidays as needed based on assignment period.
• No weekend shifts required.

Additional Information

• Floating within facility/scope of practice may be required.
• Compliance with all onboarding requirements is necessary to ensure a timely start.
