Job Summary:

The Home Health Registered Nurse (RN) is responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care will be delivered in accordance with organizational philosophy, policies, and the standards of community health practice. This role includes performing admission processes and case management responsibilities adhering to the organization's policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

Responsibilities:

• Assess patient health conditions and needs.
• Implement and manage individualized care plans.
• Provide education and support to patients and their families.
• Perform admission processes for new patients.
• Conduct case management tasks within the guidelines and practices set by the organization.
• Document all patient interactions and care activities electronically in a timely manner.
• Coordinate with interdisciplinary teams to ensure comprehensive patient care.

Qualifications:

• Minimum of 1 year of certified home care experience required.
• Minimum of 1 year of case management experience required.
• Experience must be specifically within a certified home health agency; other healthcare roles are not applicable.
• Ability to travel to assigned cases as necessary.
• Weekend rotations required approximately every 6 weeks.

Unit Description:

Positions available in multiple locations within the North Region, including Beverly, Marblehead, Salem, and Swampscott.

Additional Information:

Start date will be determined based on clearance of credentialing requirements. If not cleared by the posted start date, a rescheduled start date will be necessary.
