Description

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

This position is focused on providing high-quality care to residents in a long-term care setting. The ideal candidate will work alongside a team of healthcare professionals to ensure the well-being of patients. The role requires a commitment to following medical protocols and maintaining accurate documentation.

Responsibilities

  • Continuously observe residents to identify changes in their mental and physical health, reporting findings to the Supervisor and physician as needed.
  • Receive, transcribe, and execute physician orders accurately.
  • Administer medications and treatments, ensuring proper documentation through computerized systems.
  • Supervise and delegate tasks to Licensed Nursing Assistants (LNAs) in compliance with licensing regulations.
  • Assist in resident rounds with physicians to address medical needs and facilitate communication during reviews.
  • Provide oxygen therapy and administer feedings through gastrostomy/jejunostomy as outlined in facility policies.
  • Conduct daily unit operations, including checking appointments, monitoring staff documentation, and ensuring cleanliness and safety standards are met.
  • Manage resident admissions, discharges, and transfers, completing all required documentation.
  • Prepare and maintain 24-hour reports, consistently communicating with management regarding resident concerns and staff issues.
  • Engage with residents’ families to resolve any concerns or issues and to communicate new orders as necessary.
  • Respond to incoming calls and determine appropriate actions needed.
  • Assume responsibility for the management of assigned unit during shifts as directed by supervisory staff.
  • Ensure compliance with regulations concerning controlled medications, including documentation and counting procedures during shift changes.
  • Assist in the orientation and training of new nursing staff, addressing ongoing education needs as necessary.

Requirements

  • A minimum of three years of current experience in long-term care is required.
  • Candidates must be IV certified or be willing to obtain certification during onboarding.
  • Must be able to float to various units as needed based on experience.
  • COVID-19 vaccination is mandatory, with no declinations or exemptions accepted.
  • A valid New Hampshire nursing license must be active and in-hand at the time of submission.
  • Previous travel experience and experience in a rural or small facility setting are preferred.

The position requires the ability to work every other weekend, with no on-call responsibilities. Candidates should have their own mode of transportation, especially since the facility is located in a rural area with limited housing and public transport options.
