Job Description
We are seeking a dedicated Physical Therapist (PT) to join our Home Health team specializing in geriatrics, strokes, and orthopedics. This position involves providing therapy services to adult and geriatric patients in their homes, ensuring high-quality patient care and effective rehabilitation.
Key Responsibilities
- Conduct evaluations and develop personalized treatment plans for patients.
- Provide therapy services in accordance with established protocols and patient needs.
- Coordinate care with other healthcare professionals and provide education to patients and their families.
- Document patient progress and adjust therapy plans as necessary.
- Maintain compliance with all regulatory and organizational policies.
Qualifications
- Minimum of 1 year of physical therapy experience; first timers are accepted.
- BLS certification required.
- Valid Physical Therapist license in Georgia or Compact PT license acceptable (check member state eligibility).
- Experience with Home Care Home Base (HCHB) is highly preferred; training required for those without experience.
Work Environment
This position requires traveling to patients' homes. Mileage is compensated at the IRS standard rate. The clinician will work with a patient ratio of 7 visits per day, with a full-time productivity benchmark of approximately 25 units.
Additional Information
- Scrub Colors: Any color.
- Maximum Request Time Off (RTO): 7 days.
- Weekend duty: Not required.
- Floating within the facility/scope of practice may be required as needed.
- Compliance with all requirements necessary to start on time is mandatory.
- Holiday coverage: The traveler may be expected to work one to two major holidays during their assignment period.
Orientation
Non-billable orientation hours will be determined based on the length of the contract (e.g., 8 hours for an 8-week assignment).
