Job Description

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in a skilled nursing facility. This role is pivotal in providing high-quality care to our residents.

Responsibilities

• Administer medications and treatments as prescribed by the healthcare team.
• Monitor and document patients’ vital signs and health changes.
• Assist in the daily living activities of residents, including personal hygiene and mobility support.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Participate in care planning and provide input on care decisions.
• Ensure compliance with state regulations and facility policies.

Qualifications

• Active Colorado Nursing License.
• Long-term care experience is preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively as part of a healthcare team.
• Commitment to providing quality patient care.

Schedule

This position will involve 12-hour day shifts. Flexibility and availability for varied shifts may be required.
