Job Description

We are seeking a dedicated Registered Nurse (RN) with home care experience to provide quality clinical services to patients in their homes. The ideal candidate will have a compassionate approach to nursing and a commitment to patient education and support.

Responsibilities

• Deliver clinical services to patients ranging from infants to seniors over 100 years old.
• Assess patient conditions, implement care plans, and provide continued education and support to patients and their families.
• Manage the admissions process and oversee case management tasks in alignment with established policies and standards of practice.
• Utilize thorough and timely electronic documentation and facilitate interdisciplinary communications to ensure comprehensive patient care.

Qualifications

• Minimum of 1 year of certified home care experience.
• Minimum of 1 year of case management experience.
• Current licensure as a Registered Nurse in the state of Massachusetts.
• Reliable and independent means of transportation for home visits.
• Willingness to work weekends, holidays, and on-call shifts as needed, with weekend rotations approximately every 6 weeks.

Unit Description

This position will serve various locations within the South Region, including areas such as Allston, Belmont, Brighton, Brookline, Chestnut Hill, Newton Corner, Newtonville, Watertown, and West Newton.
