Position Overview

The Licensed Practical Nurse (LPN) is responsible for providing high-quality care to patients in a long-term care setting. This role involves collaboration with healthcare providers and support staff to ensure the delivery of optimal patient care.

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor patient health and report any changes to the healthcare team.
• Assist in the development and implementation of patient care plans.
• Provide personal care and support to residents, ensuring their comfort and dignity.
• Document patient information accurately in the electronic medical records (EMR) system.
• Collaborate with team members to enhance patient care and safety.

Qualifications

• Active and unrestricted LPN license in the applicable state.
• Minimum of 2 years of relevant experience in a long-term care environment.
• Ability to work effectively as part of a multi-disciplinary team.
• Proficient in electronic medical record (EMR) systems.
• Strong communication and interpersonal skills.

Work Schedule

This position is primarily day shifts with specific days required as outlined by the scheduling needs.

Additional Information

• This position may require floating to other units as needed.
• Parking is available at designated locations for staff.
• Scrub colors and dress code to be determined based on facility policy.
