Description
Join our team as a Registered Nurse (RN) Case Manager, focused on providing exceptional care management and discharge planning for patients. This position requires a commitment to working Monday through Friday, including every third weekend and holidays as needed.
Position Information
- Role: RN Case Manager

- Weekend Requirements: Every other weekend availability, including holidays
- Float Requirements: Ability to cover department needs as required
Key Responsibilities
- Conduct discharge planning and care coordination for patients
- Collaborate with healthcare teams to facilitate patient care
- Manage complex and high-acuity patient cases, with a focus on behavioral health
Qualifications
- Experience: Minimum of 5 years in hospital case management and 5+ years in acute adult care nursing
- Must Have Skills: Expertise in discharge planning and care coordination
- Preferred Experience: Over 5 years of nursing practice in a hospital setting
