Description

Role: LPN (Long-Term Care/Skilled Nursing)

Summary: Provides supportive/restorative nursing care to residents under RN direction; supports quality of life and works within the interdisciplinary team.

Key Duties: Direct resident care (hygiene, treatments, mobility, nutrition, safety), administers meds (with IV/blood product limits), performs approved IV therapy if IV-certified, completes treatments, admissions, and prep for tests/procedures, contributes to person-centered care plans, assists with resident/family teaching, and oversees CNA care.

Requirements: NYS LPN license (active/current), BLS (AHA) or NYS CPR, completion of an LPN program.

Preferred: 1+ year relevant experience; strong teamwork and communication skills.
