Description
American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of recent experience and proficiency with HCHB and OASIS documentation.
Details
- Home health setting serving a geriatric patient population
- Caseload of approximately 5-7 patients per day
- Schedule is Monday through Friday, days, with optional weekends
- On-call required every 6th weekend
- Territory covers up to a 60-mile radius from the hub office
- EMR used is HCHB (Homecare Homebase)
Requirements
- Minimum 1 year of recent PT experience required
- BLS certification required
- OASIS experience required for completing Start of Care documentation
- Proficiency in HCHB required
- Active driver's license and personal transportation required
- License required if applicable
Additional Information
- Responsibilities include timely documentation and communication of patient care
- Traveler must be flexible with assigned territory
- Scrubs required, any color, no jeans permitted
- iPad provided; personal cell phone not required for work use
- Mileage reimbursed at the IRS standard rate
- Monday orientation with billable training modules completed during this time
- First-time travelers will not be considered
- Local candidates are not accepted
- Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
- Travel pairs accepted, but each traveler is assigned to their own territory and must have their own transportation
- Requested time off must be submitted at the time of application
