Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring HCHB and OASIS experience along with at least 1 year of recent home health experience.

Details

Home Health setting serving the Mossyrock & Morton Area territory
Patient population primarily geriatric
Average caseload of 5-7 patients per day
Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
On-call rotation required approximately every 6th weekend
Traveler must be flexible with territory assignments
Uses HCHB (Homecare Homebase) EMR

Requirements

Minimum 1 year of recent Home Health experience required
BLS certification required
OASIS experience required for completing Start of Care (SOC) assessments
Proficiency in HCHB EMR required
Active driver's license and personal transportation required
License required if applicable
Strong documentation and communication skills required

Additional Information

Responsibilities include home exercise program development and patient/family education
Treats orthopedic and neurologic conditions including post-op surgeries, spinal conditions, stroke rehabilitation, and traumatic brain injury
Scrubs required in any color; no jeans permitted
iPad provided by facility; personal cell phone not required for work
Mileage reimbursed at the IRS standard rate
Orientation held on Mondays, including completion of billable training modules
Hospice/Home Health checklist completion is mandatory
RTO must be submitted at the time of application
First-time travelers will not be considered for this role
Local candidates will not be accepted
Travel pairs accepted, though each traveler is assigned their own territory and must have personal transportation
Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
