Description

American Traveler is seeking a Home Health Physical Therapist with 1+ years of recent HH experience and OASIS proficiency to serve rural and metro territories.

Details

• Home Health setting covering Clinton office territory (Henry, St. Clair, & Benton counties, 2x/week) and Kansas office territory (Eastern Jackson and Lafayette counties, 3x/week)
• Territories include a mix of rural counties and Kansas City metro area coverage
• Day shift schedule, 9:00 AM - 5:00 PM, 5x8 hours
• No weekend scheduling required
• 13-week contract length

Requirements

• Minimum 1 year of recent Home Health experience required
• OASIS experience required for staff completing Start of Care (SOC) documentation
• Proficiency in HCHB (Homecare Homebase) charting system required
• Active driver's license and personal transportation required
• License required if applicable
• Strong communication and documentation skills required

Additional Information

• Responsibilities include timely completion of documentation and communication with the care team
• Completion of Home Health Checklist is mandatory prior to starting
• Orientation includes completion of billable training modules
• RTO requests must be submitted at the time of application
• Travel pairs accepted, though each traveler is assigned to their own territory and must have their own transportation
• First-time travelers will not be considered for this position
