Description

American Traveler is seeking a Home Health Physical Therapist with 1+ year of skilled home health experience to serve patients in the West Portland, OR area.

Details

Home health setting covering West Portland, OR and surrounding areas
Day shift schedule, 8-hour shifts (5x8), 8:00 AM to 4:30 PM
1:1 patient ratio with up to 6 patient visits per day; 30 productivity points standard
No weekend or on-call requirements
No floating between units required
Charting completed via Home Care Home Base (HCHB), Remote Client, and Rover phone app, with Oasis documentation
Occasional daily travel radius up to 60 miles (60-90 minutes total drive time)

Requirements

License required if applicable
BLS certification required
Minimum 1 year of skilled home health experience required
Oasis experience required for staff completing Start of Care (SOC)
Proficiency in HCHB (Homecare Homebase) required
Must have personal transportation and an active driver's license

Additional Information

Responsibilities include home-based physical therapy evaluation and treatment with minimized drive time through efficient client grouping
Primary series COVID vaccination plus one booster required
Business casual attire required; scrubs allowed but not mandatory, no jeans
Monday orientation includes completion of billable training modules
Hospice/Home Health checklist completion is mandatory
Travel pairs accepted, though each traveler is assigned their own territory and must have personal transportation
Local candidates accepted within a 50-75 mile radius at the same rate
First-time travelers are not accepted for this role
Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
Requested time off must be submitted at the time of application
