Description

American Traveler is seeking a Home Health Physical Therapist with 1+ years of experience to serve patients across the Dayton, Ohio area.

Details

Full-time home health position covering Dayton, Huber Heights, Fairborn, and Yellow Springs territories
Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
HomeCare HomeBase charting experience preferred but not required

Requirements

Minimum 1 year of experience required
BLS certification required
Active OH PT license required prior to start date; cannot start with a pending license
License required if applicable
Experience with acute care setting preferred
Skilled in assessment, evaluation, discharge planning, and development of long and short term goals
Proficient with OASIS and plan of care documentation
Experience with assistive devices including crutches, gait belts, rolling walkers, standard walkers, and wheelchairs
Knowledge of home exercise programs, patient and family education, and safety and injury prevention
Familiarity with Medicaid, Medicare A, and Medicare B billing

Additional Information

Responsibilities include conducting patient assessments, identifying environmental barriers, and performing pain assessments
Focus on patient and family education for safety and injury prevention in the home setting
Open to first-time travelers
