Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring 1+ year of home health experience and current state licensure.

Details

• Home health setting providing skilled visits to patients covered by Medicare, Medicare Advantage, and commercial insurance
• Patient population includes med/surg, cardiac, ortho, hip/knee/shoulder, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trach, trauma, and amputation cases
• 6-8 home skilled visits per day with a minimum of 6 points daily, which may increase at manager's discretion
• Day shift schedule, 5x8 hours, 8:00 AM-5:00 PM
• 1-2 weekend shifts required per contract
• Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation system
• Provided with laptop, MiFi for internet access, and portable printer

Requirements

• Current BLS certification required
• Active state license required; license required if applicable
• Minimum 1 year of experience required
• Home health experience required
• OASIS training preferred
• Valid driver's license and personal vehicle required for home visits
• Must be COVID vaccinated or have exemption on file
• Must have flu shot or declination, with mandatory mask wearing at all times if declined
• Experience with assessment, cardiopulmonary/circulatory conditions, orthopedic and neurologic care, home exercise programs, patient and family education, and billing required
• Proficiency with equipment including gait belts, walkers, sit-to-stand lifts, sliding boards, cervical collars, supplemental oxygen, upper extremity prosthetics, and wheelchair assessment

Additional Information

• Main responsibilities include home-based skilled therapy visits, patient assessment, documentation, and family education
• Candidates must be willing to float within the assigned hospital system or nearby affiliate hospitals
• Any solid color scrub accepted for dress code
• Local candidates accepted at the same rate
• Former BayCare employees must wait at least 12 months before returning as a traveler
• RTO is not accepted for this contract; candidates requesting time off must be submitted for direct discussion with the hiring manager
• Cerner experience preferred
• First-time travelers not specifically addressed
