Description
American Traveler is seeking a Home Health Physical Therapist with 1+ year of experience to provide in-home care to adult and geriatric patients across multiple counties.
Details
- Home Health setting serving adults through geriatric patients
- Day shift schedule, 5x8 hours, 8:00 AM-4:30 PM
- Patient to clinician ratio of 1:5
- Travel required across 4 counties within the service area as part of daily caseload
- Mileage reimbursed at IRS standard rate and tracked through the charting system
- Floating within facility/scope of practice as needed
- No weekend shifts required
- Uses HomeCare HomeBase (HCHB) for charting, with a transition to Epic expected around September 2026
Requirements
- Minimum 1 year of experience required
- BLS certification required
- GA license or Compact PT license required (license required if applicable)
- Experience with HomeCare HomeBase (HCHB) strongly preferred; clinicians without experience must complete a paid online HCHB course prior to starting
- Skilled in assessment, documentation, and treatment planning for orthopedic, neurologic, and geriatric populations
- Experience with home health equipment such as Hoyer lifts, walkers, wheelchairs, and specialty beds
- Knowledge of OASIS documentation and home exercise program development
Additional Information
- Primary responsibilities include evaluation, treatment planning, and home-based rehabilitation for orthopedic and neurologic conditions
- Focus on patient and family education, safety, and injury prevention
- Any scrub color accepted
- Maximum of 7 days RTO per contract, finalized at time of submission
- Holiday coverage required based on number of major holidays falling within the assignment period
- First-time travelers are accepted and will be screened for a 6-month history
- Local candidates accepted at the same rate
- Full compliance required to start on time; non-compliance may delay start by two weeks due to biweekly orientation scheduling
