Description
American Traveler is hiring an RN - Home Health for adult and pediatric patients requiring a UT or compact RN license and home care experience.
Details
- Home Health setting serving adult and pediatric patients and their families in home settings
- Schedule consists of four 10-hour day shifts: Monday, Wednesday, Friday, and every other Saturday/Sunday
- Requires commuting to the office at least twice daily to electronically transfer patient files
- Uses Epic for charting
Requirements
- Active UT or compact RN license required
- Current BLS certification (AHA) that does not expire prior to start date
- ASN or nursing diploma required; BSN preferred
- 1 year of experience in home health specialty required
- Current driver's license with insured, reliable transportation and acceptable driving record
- Ability to transfer and ambulate patients independently
- Ability to lift and carry varying weights, navigate stairs and uneven surfaces, and drive a vehicle
- Strong verbal and written communication skills and basic computer skills
- DOB and last 4 of SSN required for consideration
Additional Information
- Provide comprehensive nursing care including assessment, planning, implementation, and evaluation for hospice/home health patients
- Work autonomously in patients' homes while coordinating with an interdisciplinary care team
- Deliver care in compliance with hospice/home health policies, legal, and regulatory standards
- Utilize evidence-based practice, therapeutic intervention, and technology to support patient care
- Collaborate with patients, families, physicians, and support staff to develop and adjust individualized care plans
- Permanent staff must be separated from any Intermountain facility for at least one year before being eligible for this assignment
- Allow at least 2 weeks for background check and drug screening results through a specific vendor
