Description

American Traveler is seeking a Physical Therapist for a Skilled Home Health assignment requiring at least 1 year of recent home health experience and proficiency with OASIS and HCHB documentation.

Details

Skilled Home Health setting with 1:1 patient ratio
Up to 6 patients per day with approximately 30 productivity points per week
Day shift schedule, 8-hour shifts (8:00 AM - 4:30 PM)
4x10s and 3x12s schedules not available
Weekend support may be required
No call requirement, though on-call rotation may be discussed during interview
Documentation completed using Home Care Home Base (HCHB) and OASIS
Utilizes Remote Client for Home Health on laptop and Rover phone application

Requirements

BLS certification required
Minimum 1 year of skilled Home Health experience required
OASIS experience required for completing Start of Care (SOC) documentation
Proficiency in Home Care Home Base (HCHB) required
Active driver's license and reliable personal transportation required
License required if applicable

Additional Information

Facility makes efforts to group patient visits and minimize driving
Service area may extend up to approximately 60 miles or 60-90 minutes of total daily driving
Mileage reimbursed at current IRS standard rate
Business casual attire required; no jeans; scrubs permitted but not required
Free parking available
Up to 3 days of RTO may be automatically approved
One holiday approval available among Thanksgiving, Christmas, or New Year's
First-time travelers accepted
Local clinicians accepted within 50-75 mile radius at the same rate
Travel pairs accepted, though each traveler is assigned to their own territory and must have their own transportation
Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
