Description

American Traveler is hiring a Home Health RN for a skilled Home Health assignment requiring active Oregon and Washington licensure and HCHB and OASIS experience.

Details

• Skilled Home Health setting with 1:1 patient ratio
• Up to 6 patients per day with approximately 30 productivity points per week
• Day shift schedule, 8-hour shifts
• Weekend support may be required
• On-call rotation may be required, to be discussed during interview
• Service area may extend up to approximately 60 miles or 60-90 minutes of total driving per day
• Uses Home Care Home Base (HCHB), Remote Client for Home Health, Rover phone application, and OASIS documentation

Requirements

• Active OR and WA RN license required prior to start date; temporary licenses not accepted
• Current BLS certification required
• Minimum of 1 year skilled Home Health experience required
• OASIS experience required for Start of Care (SOC) documentation
• Proficiency in HCHB (Homecare Homebase) required
• Must have reliable personal transportation and an active driver's license

Additional Information

• Responsibilities include admission assessments, recertifications, resumption of care, routine visits, and start of care documentation
• Performs wound care, medication administration and management, pain assessment, IV and subcutaneous administration, and phlebotomy as needed
• Facility works to group patient visits to minimize driving time
• Business casual attire required; no jeans; scrubs permitted but not mandatory
• Mileage reimbursed at current IRS standard rate; free parking provided
• Monday orientation required, including completion of billable training modules
• Local clinicians accepted within a 50-75 mile radius at the same rate
• First-time travelers accepted only with prior Home Health experience
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Travel pairs accepted, but each traveler is assigned their own territory and must have their own transportation
• Requested time off (RTO) must be submitted at the time of application
