Description

American Traveler is seeking a Home Health Physical Therapist with 1+ year of home care experience to provide in-home patient care across the North Region.

Details

Home health setting providing in-home physical therapy services in Chelsea, East Boston, Revere, and Winthrop
Patient population ranges from infants to centenarians with physical impairments, functional limitations, or disabilities
Collaborates with patients, families, physicians, and allied health professionals to coordinate care
Shift schedule 08:00-16:30
Weekend rotation once every 6 weeks
Requires travel between patient homes as assigned
Uses Epic for charting
13-week assignment

Requirements

Active MA Physical Therapist license required (license required if applicable)
Minimum 1 year of home care experience required
Ability to travel to multiple patient locations as part of caseload
Strong skills in electronic documentation and interdisciplinary communication
Flu vaccination required with no religious or medical exemptions

Additional Information

Provide comprehensive physical therapy evaluation and treatment in patients' homes
Maintain timely and thorough documentation in accordance with agency policies and standards of practice
Start date contingent on credentialing clearance; delays may result in a rescheduled start
Local candidates considered, with a 60-mile radius rule from Boston affecting rate eligibility
Timekeeping managed through UKG/Kronos
Parking at Boston-area locations is not validated; public transportation or paid lots required
