Description
American Traveler is hiring an RN for a Home Health position requiring a Maine or compact RN license and 2 years of nursing experience.
Details
- Home Care department delivering skilled nursing services to patients in their homes
- 13-week assignment with day shift hours from 8:00 AM to 5:00 PM
- Epic EMR used for documentation
- 25-mile float policy between facilities as needed
Requirements
- Active RN license in Maine, New Hampshire, or compact required
- CPR certification required
- Minimum 2 years of nursing experience required
- Driver's license required for consideration
- 2 professional references required
Additional Information
- Evaluate patients using subjective and objective findings per clinical standards of practice
- Establish and regularly update goal-oriented nursing plans of care in collaboration with providers
- Administer nursing interventions and counsel patients/families on self-care and preventive health measures
- Coordinate care with other professional services and communicate patient status to attending physicians as needed
- Participate in case conferences and team meetings, and manage assigned patient caseload
- Establish clear discharge plans and make referrals to outpatient services as needed
- Apply Medicare Home Health Benefit concepts regarding skilled care and homebound requirements
- Local candidates within 90 miles of any MaineHealth facility are eligible but not eligible for renewals or extensions
- RTO should be minimal, with 3-4 days considered reasonable for the 13-week contract
- Candidates who worked at any MaineHealth facility within the past 12 months should disclose this upon consideration
