Description

American Traveler is hiring a Home Health RN with skilled home health experience, HCHB proficiency, and active Oregon and Washington licensure to provide 1:1 patient care in a skilled home health setting.

Details

• Skilled Home Health setting providing in-home patient care
• Patient ratio of 1:1 with up to 6 patients per day
• Approximately 30 productivity points per week
• Day shift schedule, 8-hour shifts (8:00 AM to 4:30 PM)
• Weekend support may be required
• No on-call requirement specified in assignment details, though on-call rotation may be discussed during interview
• Service area may extend up to approximately 60 miles or 60-90 minutes of total driving per day
• Uses Home Care Home Base (HCHB) for documentation
• Uses Remote Client for Home Health on a laptop and Rover phone application
• OASIS documentation required for Start of Care (SOC) visits

Requirements

• Active Oregon and Washington RN license required and in-hand before start date; temporary licenses not accepted
• Current BLS certification required
• Minimum of 1 year of skilled Home Health experience required
• OASIS experience required for staff completing Start of Care (SOC)
• Proficiency in Home Care Home Base (HCHB) required
• Reliable personal transportation and active driver's license required
• Completion of Hospice/Home Health Checklist mandatory

Additional Information

• Provide skilled home health nursing care with visits grouped to minimize driving time
• Monday orientation required, including completion of billable training modules
• Business casual attire required; no jeans; scrubs permitted but not mandatory
• Free parking available; mileage reimbursed at the current IRS standard rate
• Up to 3 days of RTO may be automatically approved
• Blackout periods apply during the weeks of Thanksgiving, Christmas, and New Year's
• Local clinicians accepted within a 50-75 mile radius at the same rate
• Travel pairs accepted, though each traveler is assigned their own territory and must have personal transportation
• Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
• Timely communication and documentation are mandatory
