Description
American Traveler is seeking a Physical Therapist for a Skilled Home Health assignment, requiring at least 1 year of skilled Home Health experience and proficiency in HCHB and OASIS documentation.
Details
- Skilled Home Health setting with a 1:1 patient ratio
- Day shift schedule, 8-hour shifts (8:00 AM - 4:30 PM)
- 4x10s and 3x12s schedules are not available
- Approximately 30 productivity points per week, up to 6 patients per day
- Weekend support may be required
- On-call rotation may be required
- Uses Home Care Home Base (HCHB), Remote Client for Home Health, Rover phone application, and OASIS documentation
Requirements
- Minimum 1 year of skilled Home Health experience required
- BLS certification required
- OASIS experience required for completing Start of Care (SOC)
- Proficiency in Home Care Home Base (HCHB) required
- Active driver's license and reliable personal transportation required
- License required if applicable
- Hospice/Home Health Checklist completion required
Additional Information
- Facility groups patient visits to minimize driving, though service area may occasionally extend up to 60 miles or 60-90 minutes of total daily driving
- Business casual attire required; no jeans; scrubs permitted but not mandatory
- Free parking available
- Mileage reimbursed at current IRS standard rate
- Up to 3 days of RTO may be automatically approved
- One holiday approval available among Thanksgiving, Christmas, or New Year's
- Monday orientation includes billable training modules
- First-time travelers are not accepted
- Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
- Travel pairs accepted, but each traveler is assigned their own territory and must have their own transportation
- Local clinicians accepted within a 50-75 mile radius at the same rate
