Description

American Traveler is seeking a Home Health Physical Therapist with 1+ year of recent experience and HCHB EMR proficiency to cover a geriatric caseload in a home health territory.

Details

Home Health setting covering a territory within a 60-mile radius of the hub office
Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
Average caseload of 5-7 patients per day
Patient population consists of geriatric home health cases
Uses HCHB (Homecare Homebase) EMR system
On-call rotation required approximately every 3rd to 4th weekend
Traveler must remain flexible with assigned territory

Requirements

Minimum 1 year of recent Physical Therapy experience required
BLS certification required
OASIS experience required for completing Start of Care (SOC) documentation
Proficiency in HCHB EMR required
License required if applicable
Active driver's license and personal transportation required
Must provide timely documentation and communication

Additional Information

Position covers an FMLA leave starting in February
Scrubs required in any color; jeans not permitted
No cell phone provided; iPad provided for work use
Mileage reimbursed at the IRS standard rate
Monday orientation required, including completion of billable modules
Hospice/Home Health checklist completion is mandatory
RTO requests must be submitted at the time of application
First-time travelers are not accepted for this position
Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
Travel pairs accepted, though each traveler will be assigned their own territory and must have personal transportation
Local candidates accepted at the travel rate
