Description

American Traveler is hiring a Home Health RN with a VT or compact RN license and at least 2 years of experience to manage patient caseloads in the community setting.

Details

• Home health setting providing care to patients in their residences
• Serves as Case Manager for assigned patients, managing caseloads and visit times per agency productivity standards
• M-F schedule from 8am-4pm with some weekend work required
• On-call responsibilities required, including hospice coverage during on-call periods
• Holiday coverage required

Requirements

• Active VT or compact RN license required upon acceptance
• Minimum of 2 years of nursing experience required
• Experience with high-tech skills such as IV therapy, wound VAC, and complex wound care
• Familiarity with OASIS documentation preferred
• Experience with McKesson charting system preferred

Additional Information

• Conduct comprehensive patient and caregiver assessments using the nursing process to develop individualized care plans
• Implement medical plans of care, nursing interventions, and procedures to ensure safe and effective patient outcomes
• Collaborate with LPNs, delegating tasks and reviewing documentation to ensure continuity of care
• Provide patient and caregiver education on medications, treatment options, discharge instructions, and reportable conditions
• Coordinate interdisciplinary care with physicians, providers, and other team members to support the plan of care
• Complete required agency, state, and federal forms in a timely manner
• Participate in monthly peer review audits and case conferences with the care team
• Coordinate safe patient discharges and transfers in accordance with regulations
• Maintain accurate and up-to-date nursing documentation to support continuity of care
